A home and community-based pathway for eligible children.

Personal Assist is approved by the State of North Carolina to provide CAP/C services for eligible children with an authorized plan.

What is CAP/C?

The Community Alternatives Program for Children, commonly called CAP/C, is an NC Medicaid home and community-based services program for eligible medically fragile or medically complex children. The program is designed to help approved beneficiaries remain in or return to a home and community setting when they would otherwise be at risk for institutional care.

NC Medicaid describes CAP/C as serving eligible children from birth through age 20 who meet the program’s Medicaid, level-of-care, needs-assessment, and other requirements. A child matching a general description is not automatically eligible.

Personal Assist’s role

Personal Assist is state-approved to provide CAP/C services. When a child has an authorized plan that includes services we provide, our team can work with the family, case manager, and other appropriate contacts to clarify the approved tasks, schedule, county, and staffing needs.

A family-centered conversation

Families know their child’s routines, communication, comfort, and daily needs. We listen carefully while following the approved service plan and program requirements. The goal is consistent, respectful support in the child’s familiar home and community environment.

Helpful information when you call

  • The child’s county and age
  • Whether a CAP/C referral, assessment, or authorization exists
  • The assigned case manager or case-management entity, if applicable
  • The services and schedule included in the authorized plan
  • The family’s preferred contact information and general timing needs

Where to start

Families already participating in CAP/C can call us to discuss provider coordination. Families new to the program can review the State’s official CAP/C information and referral guidance. Do not place medical records, diagnoses, Medicaid numbers, or other confidential information in the website form.

Does Personal Assist determine CAP/C eligibility?

No. Eligibility and authorization are determined through the official program process.

Does approval as a provider guarantee staffing?

No. We must also confirm the service plan, location, requested schedule, and available qualified staff.

Personal Assist does not determine eligibility or guarantee authorization, coverage, an immediate start date, or service availability.
